Licensing - Common Application Errors
Applicants frequently submit incomplete or incorrect applications, which results in processing delays, or returning of the incomplete application. It is the responsibility of the applicant’s authorized representative to ensure the application is accurate, complete, and properly supported with all required documentation before submission.
Most Common Reasons for Processing Delays
Missing or Incorrect Board Resolution
Board or member resolution not attached.
Authorized signer not listed on the resolution.
Insurance Certificate Issues:
Not submitted.
Not current.
LMVC is not listed as certificate holder (must be: 3017 Kingman Street, Metairie, LA 70006).
Payment Errors:
Incorrect fee amount.
Check not payable to “Louisiana Motor Vehicle Commission.”
Bank account does not match applicant.
Cannot accept a personal check: a business check, money order or cashier’s check is acceptable
Application Form Problems:
Altered applications (white-outs, scratch-outs, cut-offs).
Photocopies submitted instead of originals with original signatures.
Missing Required Documents or Information:
Not registered or “not in good standing’ with the Louisiana Secretary of State.
Additional requested data\documentation per the LMVC instructions not submitted.
Criminal history section incomplete for salesperson, agent, or representative license applications:
- If “Yes” is indicated, full details of arrests/convictions must be provided.
- Final court judgments must be included for any convictions.
- Prior license denials / suspensions / revocations not disclosed or explained.
Omissions Related to Background Disclosures of applicant, any partner, corporate stockholder, L.P. member or manager, any L.L.P. member or manager, or any L.L.C. member or manager, director or officer of said applicant:
Failure to report or provide full documentation for:
- Arrests, charges, or convictions (within 10 years).
- Prior occupational license denials or disciplinary actions.
- Bankruptcies (requires explanation letter and supporting documents).
Bond Form Errors:
Bond not made payable to Secretary of the Department of Public Safety and Corrections.
Bond not covering full license period.
Bond unsigned by principal or surety representative.
Missing power of attorney authorizing bond signer.
Incorrect bond form submitted.
